Foigo.com allows you to create customizable forms, databases, or web surveys. You can create forms with various elements such as checkboxes, radio buttons, drop-down menus, or text fields, and this site will handle writing the code for you. New users can choose between a detailed tutorial illustrated by screenshots, video tutorials (unfortunately, these don’t seem to have sound), or can jump right in for a “test drive” using the test account.
